In our efforts to provide the highest quality care for our patients, Community Home Health & Hospice has made the decision to partner with Eden Health to ensure the continuity of care for our Home Health and Hospice patients.
How many staff will be affected by this buyout?
Eden has offered to onboard Community’s clinical staff and many administrative staff.
Why did Community choose Eden?
Eden is a 100% employee-owned company that is patient-focused and has similar core values to Community Home Health & Hospice. They are committed to continuing to provide the highest level of quality care to the residents of Clark, Cowlitz, Columbia, and Wahkiakum counties.
How will this affect patients currently on service?
Patients and their families should expect little to no disruption in their current care plan. Their care team will remain virtually unchanged aside from the organization they work under starting May 1.
Is Eden acquiring the care centers?
No, Eden’s working model does not include hospice care centers. Community Home Health & Hospice is exploring avenues to keep the hospice care centers operational. Community is working with various community partners to find a path forward that keeps the care centers open.
